All providers are NHS-approved, follow NICE guidelines, and can assess, diagnose, and treat ADHD.

- Complete the Questionaire:
- Return both the completed provider form(s) and Questionnaire to the surgery
What happens if I’m diagnosed?
The specialist will start treatment. Once stable, your GP may prescribe under a shared care agreement.
Shared Care
Your GP can prescribe if you attend annual specialist reviews and 6-monthly checks (BP, pulse, weight, height). The specialist manages medication changes.
Important Information
GPs cannot change medication doses. Prescriptions may stop if reviews or monitoring are missed.

Private Diagnoses
We cannot accept shared care for private-only ADHD diagnoses.
